In the United States, individuals aged 65 and above constitute 62% of annual incident dialysis patients, according to the USRDS Renal Data Extraction and Referencing System. It is noteworthy that approximately 7% of end-stage renal disease (ESRD) patients currently reside in skilled nursing facilities, although this figure is likely an underestimate (in 2015-2017, as reported by the Center for Clinical Standards and Quality).
Since 2022, we have collaborated with the Department of Health (DOH) to introduce a care model notably absent in the state of Washington. On November 13, 2023, we announced the first day of operation for a much-needed transformation in the care provided to our ESRD patients receiving maintenance dialysis in skilled nursing facilities. It marked the initiation of in-facility (SNF/NF) dialysis.
Our partnership with Regency Park Rose and Outset has turned our dream into reality. We believe that this model will reduce the burden on SNF/NF for the care of ESRD residents and lower costs for the medical system as a whole. This approach aims to improve the quality of life for our dialysis residents by providing comprehensive care for ESRD patients undergoing maintenance dialysis. Understanding the challenges our ESRD patients face, we focus on alleviating daily obstacles associated with dialysis. We seamlessly integrate dialysis into the facility's daily activities, minimizing interference with short- or long-term care goals, making them more attainable. Collaborating with innovative experts in ESRD dialysis care, we offer a comprehensive approach to caring for our ESRD patients.
Our nephrologists and nephrology team communicate with our nursing staff, dietitians, and social workers to provide personalized care tailored to each individual's needs.
